Surgical interventions are aimed at palliative treatment in most patients with metastatic skeletal lesions. However, curative resections with reconstruction and plasty steps may be considered in selected cases of a solitary metastatic lesion. The life expectancy prognosis based on the histological structure of the tumor is a significant and decisive factor in choosing the appropriate surgery.</p> <p><bold>Aim</bold>. To develop a prognostic model for choosing surgical treatment for metastatic bone lesions.</p> <p><bold>Materials and methods</bold>. Treatment analysis of 715 patients with a history of surgery for metastatic bone lesions of various localizations is presented. A total of 780 surgical interventions were performed. Surgeries for the complications of bone metastases were mainly performed on the spine (48.5% of all surgeries), followed by long bones with 247 (35%) surgeries, pelvic bones with 81 (11%) interventions, and the chest wall with 40 (5.5%) surgeries.</p> <p><bold>Results</bold>. The most unfavorable prognostic factors in patients with metastatic bone lesions are the histological type of the primary tumor of the rapid growth group (risk ratio [RR]=5.11), visceral metastases (RR=3.1), Charlson Comorbidity Index over 10 (RR=3.07) and presence of critical laboratory abnormalities (RR=2.91), as they have the highest rates of impact on survival (over 2.9).</p> <p><bold>Conclusion</bold>. The developed 14-point mathematical score of life expectancy prognosis, which includes five oncological and four clinical factors, defines with an accuracy of 91% the risk groups of good (estimated life expectancy over one year), moderate (6 to 12 months), and poor (less than six months) prognosis in patients with metastatic bone lesions.</p></abstract><trans-abstract xml:lang="ru"><p>Определение тактики хирургического лечения пациентов с метастатическим поражением костей – одна из важных проблем современной онкологии. У большинства пациентов с метастатическим поражением скелета хирургические вмешательства направлены на паллиативное лечение, однако в отдельных случаях, при солитарном метастатическом поражении, могут быть рассмотрены радикальные резекции с реконструктивно-пластическим компонентом, а прогноз предполагаемой продолжительности жизни, связанный с гистологической структурой злокачественной опухоли, является основным и решающим фактором при определении подходящего типа хирургического лечения.</p> <p><bold>Цель</bold>. Создать прогностическую модель для выработки тактики хирургического лечения при метастатическом поражении костей.</p> <p><bold>Материалы и</bold> <bold>методы</bold>. Представлен анализ лечения 715 пациентов, которым выполнены хирургические вмешательства по поводу метастатического поражения костей различных локализаций. Всего выполнено 780 хирургических вмешательств. Наиболее часто вмешательства по поводу осложненного течения метастатического процесса в костях выполняли на позвоночнике, что составило 48,5% всех операций. На 2-м месте по частоте – длинные кости – 247 (35%) операций, на 3-м – кости таза – 81 (11%) вмешательство, и на грудной стенке выполнено 40 (5,5%) операций.</p> <p><bold>Результаты</bold>. Наиболее неблагоприятными прогностическими факторами у пациентов с метастатическим поражением костей являются гистологический тип первичной опухоли из группы быстрого роста (коэффициент риска – КР=5,11), висцеральные метастазы (КР=3,1), индекс коморбидности по Чарлсону более 10 (КР=3,07) и наличие критических отклонений в лабораторных показателях (КР=2,91), поскольку имеют самые высокие коэффициенты влияния на выживаемость (более 2,9).</p> <p><bold>Заключение</bold>. Разработанная четырнадцатибалльная математическая шкала прогноза ожидаемой продолжительности жизни, включающая 5 онкологических и 4 клинических фактора, позволяет с точностью до 91% определить группы риска хорошего (предполагаемая продолжительность жизни более 1 года), умеренного (от 6 до 12 мес) и плохого (менее 6 мес) прогноза у пациентов с метастатическим поражением костей.</p></trans-abstract><kwd-group xml:lang="en"><kwd>surgical treatment</kwd><kwd>bone metastases</kwd><kwd>cancer prognosis</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>хирургическое лечение</kwd><kwd>метастазы в кости</kwd><kwd>онкологический прогноз</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Бухаров А.В, Алиев М.Д., Державин В.А., Ядрина А.В.
