This event brought together a multidisciplinary group of specialists, including urologic oncologists, radiologists, and chemotherapists from leading medical centers across Russia. Emphasis was placed on the necessity of standardizing positron emission tomography/computed tomography (PET/CT) utilizing prostate-specific membrane antigen (PSMA) as a critical instrument for identifying mCRPC patients suitable for RNT. Participants highlighted that enhancing accessibility to and standardizing PET/CT with PSMA for mCRPC patients are essential to the safer and more effective use of RNT, particularly for 225Ac-based radioconjugates. Achieving anticipated outcomes requires a coordinated strategy for patient selection and management, along with a well-structured patient routing process across medical centers to mitigate the risk of late complications and toxicity. If these proposed strategies are implemented, it is anticipated that the proportion of clinically significant responses will increase and that disease management outcomes will improve.</p></abstract><trans-abstract xml:lang="ru"><p>10 декабря 2025 г. в г. Санкт-Петербурге состоялось заседание Экспертного совета по радионуклидной терапии (РНТ) и роли α-эмиттеров (225Ac) при метастатическом кастрационно-резистентном раке предстательной железы (мКРРПЖ), объединившее профильных специалистов (онкоурологов, радиологов и химиотерапевтов) из ведущих центров России. Внимание уделено стандартизации позитронно-эмиссионной томографии в сочетании с компьютерной томографией (ПЭТ/КТ) с простатическим специфическим мембранным антигеном (ПСМА) как ключевого инструмента подбора пациентов с мКРРПЖ для проведения РНТ. По мнению участников совета, расширение доступа и стандартизация ПЭТ/КТ с ПСМА при мКРРПЖ является ключевым условием для более эффективного и безопасного применения РНТ, в т.ч. с использованием радиоконъюгатов на основе 225Ac. Реализация ожидаемых эффектов требует согласованных подходов к отбору и ведению пациентов и налаженной маршрутизации пациентов между центрами, что позволит снизить долю поздних осложнений с высоким риском токсичности.
