It was dedicated to the diagnostics and treatment of patients with hormone-dependent HER2-negative advanced breast cancer (HR+ HER2- aBC) with a PIK3CA mutation using target drug Piqray (alpelisib). Experts-oncologists discussed the diagnostics of the PIK3CA mutation, data from clinical studies of alpelisib, characteristics of patients who may benefit from the alpelisib, prevention and control of adverse events that may develop during therapy with alpelisib. For planning anticancer therapy using alpelisib in combination with fulvestrant, all patients with HR+ HER2- aBC should be tested for PIK3CA mutation at the first signs of a metastatic process. Data from clinical trials demonstrate that this combination provides approximately 2-fold increase in median progression free survival compared to a placebo + fulvestrant combination. Alpelisib can be prescribed to patients with HR+ HER2- aBC with a PIK3CA mutation after progression on endocrine therapy, in a combined mode. The main adverse events – hyperglycemia and rash – are associated with the mechanism of alpelisib action, and are predictable and manageable.</p></abstract><trans-abstract xml:lang="ru"><p>В рамках Большой конференции RUSSCO «Рак молочной железы» 29 января 2021 г. состоялся сателлитный симпозиум компании «Новартис Фарма», посвященный вопросам диагностики и лечения пациентов с гормонозависимым HER2-отрицательным распространенным раком молочной железы (HR+ HER2- рРМЖ) с мутацией гена PIK3CA с использованием таргетного препарата Пикрэй (алпелисиб). Эксперты-онкологи обсудили диагностику мутации PIK3CA, данные клинических исследований алпелисиба, характеристики пациентов, которым назначение алпелисиба может принести наибольшую пользу, профилактику и контроль нежелательных явлений, которые могут развиться на фоне терапии препаратом алпелисиб. Определение мутации гена PIK3CA целесообразно проводить всем пациентам с HR+ HER2- рРМЖ при выявлении первых признаков метастатического процесса, чтобы иметь возможность планировать противоопухолевую терапию с применением алпелисиба в комбинации с фулвестрантом. Данные клинических исследований демонстрируют, что такая комбинация обеспечивает практически двукратное увеличение медианы выживаемости без прогрессирования болезни по сравнению с комбинацией плацебо с фулвестрантом. Алпелисиб может быть назначен пациентам с HR+ HER2- рРМЖ с мутацией гена PIK3CA после прогрессирования на фоне применения моногормонотерапии или комбинированной гормонотерапии.
