After the introduction of the sentinel lymph node (SLN) concept into clinical practice, it became possible to remove only one LN through which lymph drainage from the mammary gland occurs and to assess its status during urgent and planned histological examination. At the current stage of oncology and radiological diagnostic method development, SLNs are primarily detected using isotopic and fluorescent drugs. The most promising areas include the use of the SLN detection technique with an ultrasonic sulfur hexafluoride contrast medium. This technique is being gradually integrated into clinical practice due to its high diagnostic value; however, it is necessary to detail its diagnostic capabilities, develop utilization algorithms, and establish specific indications for its use.</p> <p><bold>Aim.</bold> To study the possibilities of using an ultrasound contrast medium, sulfur hexafluoride, in the SLN detection, to identify the main enhancement patterns, and to determine the contrast uptake features of SLN with metastatic lesions in patients with BC.</p> <p><bold>Materials and methods.</bold> The prospective study was conducted at the Department of Breast Pathology and the Imaging Department of the Kulakov National Medical Research Center for Obstetrics, Gynecology, and Perinatology from May to October 2024. This part of the study included 144 patients (91 with stage I surgery and 53 after neoadjuvant chemotherapy) with primary resectable stage I-IIIa BC (cT<sub>1-3</sub>N<sub>0-1</sub>M<sub>0</sub>), the average age was 52.4 years. The detection technique using ultrasonic imaging was performed with paraareolar and intradermal injections of the sulfur hexafluoride contrast medium on the affected side. Imaging with contrast enhancement was performed on the Mindray Resona 7 Diagnostic Ultrasound System (PRC) with contrast function.</p> <p><bold>Results.</bold> The study detected SLN in 137 cases (out of 144), resulting in a detection rate of 95.1%. In most cases, 1 SLN was detected; in 24 (17.5%) patients, 2 SLNs were detected. In 7 (4.9%) cases, no SLN was detected (false negative results). There were 6 types (patterns) of contrast medium uptake: Type I (homogeneous, intensive), Type II (homogeneous, low-intensity), Type III (heterogeneous distribution of ultrasonic contrast medium with perfusion defects of &lt;50% of the LN volume), Type IV (heterogeneous contrast enhancement with perfusion defects of &gt;50% of the LN volume), Type V (annular), Type VI (contrast-enhanced lymphatic duct, without ultrasonic contrast medium uptake in the LN; LN structures are visualized in B mode). The most common patterns in metastatic disease were IV, V, and VI. Patterns I, II, and III were typical for LN with no signs of metastasis.</p> <p><bold>Conclusion.</bold> Detection of SLN using ultrasound contrast-enhancement technique is an easily available and reproducible method in clinical practice in patients with BC, and the assessment of the intensity of the accumulation and distribution of contrast medium with ranking by selected patterns may indicate the presence of metastases in the LN.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование.</bold> Для персонализированного подхода и выбора тактики лечения больных раком молочной железы (РМЖ) важной задачей остается правильное стадирование опухолевого процесса, в том числе оценка статуса регионарных лимфатических узлов (ЛУ). После внедрения в клиническую практику концепции сигнального ЛУ (СЛУ) стали возможными удаление только 1-го ЛУ, через который осуществляется лимфоотток от молочной железы, и оценка его статуса при проведении срочного и планового гистологического исследования. На современном этапе развития онкологии и лучевых методов диагностики детекция СЛУ проводится в основном с применением изотопных и флюоресцентных препаратов. К наиболее перспективным направлениям относится методика детекции СЛУ с применением ультразвукового контрастного препарата гексафторида серы. Эта методика постепенно интегрируется в клиническую практику за счет высоких диагностических показателей, но необходима детализация диагностических возможностей этого способа детекции, отработка алгоритмов выполнения, определение показаний к применению.</p> <p><bold>Цель.</bold> Изучить возможности применения ультразвукового контрастного препарата гексафторида серы в детекции СЛУ, выявить основные паттерны контрастирования, определить особенности контрастирования СЛУ с метастатическим поражением у больных РМЖ.</p> <p><bold>Материалы и методы.</bold> Исследование проведено на базе отделения патологии молочной железы и отдела визуальной диагностики ФГБУ «НМИЦ АГП им. акад. В.И. Кулакова» с мая по октябрь 2024 г. В данную часть исследования включены 144 пациентки (91 – с операцией на I этапе и 53 – после неоадъювантной химиотерапии) с первично операбельным РМЖ I–IIIa-стадии (сT<sub>1–3</sub>N<sub>0–1</sub>M<sub>0</sub>), средний возраст – 52,4 года. Методика детекции с ультразвуковым контрастированием выполнялась с использованием препарата гексафторида серы, который вводили параареолярно, интрадермально на стороне поражения. Визуализация контрастирования проводилась на диагностической ультразвуковой системе Mindray Resona 7 (КНР) с функцией contrast.</p> <p><bold>Результаты.</bold> В ходе исследования удалось выявить СЛУ в 137 случаях (из 144), коэффициент выявляемости составил 95,1%. В большинстве случаев определялся 1 СЛУ, у 24 (17,5%) пациенток выявлено 2 СЛУ. В 7 (4,9%) случаях СЛУ не выявлен (ложноотрицательные результаты). Выделено 6 типов (паттернов) накопления: I тип (гомогенное, интенсивное), II тип (гомогенное, слабо-интенсивное), III тип (гетерогенное распределение ультразвукового контраста, дефекты перфузии занимают &lt;50% объема ЛУ), IV тип (гетерогенное контрастирование, дефекты перфузии занимают &gt;50% объема ЛУ), V тип (кольцевидный), VI тип (контрастированный лимфатический проток, без поступления ультразвукового контраста в ЛУ, в В-режиме визуализируются структуры ЛУ). Наиболее характерными паттернами при метастатическом поражении оказались IV, V и VI. Паттерны I, II, III характерны для ЛУ без признаков метастазирования.</p> <p><bold>Заключение.</bold> Детекция СЛУ с применением методики ультразвукового контрастирования является доступной, воспроизводимой в клинической практике у пациенток с РМЖ, а оценка особенностей интенсивности накопления и распределения контраста с ранжированием по выделенным паттернам помогает предположить наличие метастатического процесса в ЛУ.</p></trans-abstract><kwd-group xml:lang="en"><kwd>sentinel lymph node</kwd><kwd>ultrasound contrast agent</kwd><kwd>breast cancer</kwd><kwd>enhancement patterns</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>сигнальный лимфатический узел</kwd><kwd>ультразвуковой контрастный препарат</kwd><kwd>рак молочной железы</kwd><kwd>паттерны контрастирования</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Sun J, Mathias BJ, Laronga C, et al.
