This emphasizes the relevance of finding predictor markers of high sensitivity to immunotherapy. Since atezolizumab acts through the local immune system, assessing its status prior to therapy administration may be crucial.</p> <p><bold>Aim.</bold> The objective of this study was to evaluate the predictor significance of immunological parameters of tumour microenvironment in patients with metastatic triple-negative cancer receiving therapy with atezolizumab in combination with nab-paclitaxel.</p> <p><bold>Materials and methods.</bold> Eleven patients with metastatic triple-negative breast cancer, who received atezolizumab in combination with nab-paclitaxel, were included in the study. Among them, five patients exhibited a durable response (progression-free survival &gt;6 months), while six patients had a short response (progression-free survival ≤6 months). Tumour microenvironment parameters (the proportion of immune cells in the microenvironment and their expression of PD-L1 and PD-1) were analyzed by TSA-associated multiplex immunofluorescence (Tyramide Signal Amplification) using the Vectra® 3.0 tissue analysis system and InForm® software (Akoya Bio, USA).</p> <p><bold>Results.</bold> The study revealed that CD163<sup>+</sup>-macrophage counts below 6.475% of all tumor stromal cells were associated with long-term efficacy of atezolizumab in combination with nab-paclitaxel in patients with metastatic triple-negative cancer.</p> <p><bold>Conclusion.</bold> The number of CD163<sup>+</sup>-macrophages in the tumor stroma could potentially serve as a marker for predicting the efficacy of atezolizumab in combination with nab-paclitaxel in patients with metastatic triple-negative cancer.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование.</bold> Химиоиммунотерапия комбинацией атезолизумаба и наб-паклитаксела демонстрирует клиническую пользу в течение 12 мес лишь у 45,8% больных прогрессирующим трижды негативным раком молочной железы (ТНРМЖ) с экспрессией PD-L1. Это подчеркивает актуальность поиска маркеров-предикторов высокой чувствительности к иммунотерапии. Поскольку механизмы действия атезолизумаба вовлекают местную иммунную систему, то важным может оказаться ее состояние до назначения терапии.</p> <p><bold>Цель.</bold> Оценить предикторную значимость иммунологических параметров опухолевого микроокружения у больных метастатическим ТНРМЖ, получающих терапию атезолизумабом в комбинации с наб-паклитакселом.</p> <p><bold>Материалы и методы.</bold> В исследование включены 11 больных метастатическим ТНРМЖ, получивших лечение атезолизумабом в комбинации с наб-паклитакселом. У 5 пациентов наблюдался длительный ответ (период клинической эффективности &gt;6 мес), а у 6 – короткий (период клинической эффективности ≤6 мес). Параметры опухолевого микроокружения (доли иммунных клеток в микроокружении и экспрессия ими PD-L1 и PD-1) анализировались с помощью TSA-ассоциированной мультиплексной иммунофлуоресценции (Tyramide Signal Amplification) с использованием системы анализа тканей Vectra® 3.0 и программного обеспечения InForm® (Akoya Bio, США).</p> <p><bold>Результаты.</bold> Показано, что количество CD163<sup>+</sup>-макрофагов ниже 6,475% всех клеток стромы опухоли ассоциировано с длительной клинической эффективностью атезолизумаба в комбинации с наб-паклитакселом у пациентов с метастатическим ТНРМЖ.</p> <p><bold>Заключение.</bold> Доля CD163<sup>+</sup>-макрофагов в строме опухоли может являться предиктором эффективности терапии атезолизумабом в комбинации с наб-паклитакселом у пациентов с метастатическим ТНРМЖ.</p></trans-abstract><kwd-group xml:lang="en"><kwd>breast cancer</kwd><kwd>microenvironment</kwd><kwd>atezolizumab</kwd><kwd>CD163+-macrophages</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>рак молочной железы</kwd><kwd>микроокружение</kwd><kwd>атезолизумаб</kwd><kwd>CD163+-макрофаги</kwd></kwd-group><funding-group><funding-statement xml:lang="en">This work was supported by the Russian Science Foundation (grant No. 20-75-10033-П).
