Заболеваемость В-ХЛЛ составляет 3 случая на 100 000 населения в год. За последние 10 лет в терапии В-ХЛЛ произошел значительный прогресс. Он связан с появлением новых препаратов – аналогов пуринов и моноклональных антител – и с разработкой эффективных комбинированных режимов. Цель настоящих рекомендаций – выработать современную тактику обследования и ведения больных В-ХЛЛ в России с учетом изменившихся терапевтических возможностей. Рекомендации основываются на изучении ключевых работ по терапии В-ХЛЛ, в том числе метааналитических исследований, а также на анализе отечественного опыта лечения В- ХЛЛ. Данная версия руководства подготовлена инициативной российской группой по изучению В-ХЛЛ. В основу концепции терапии В-ХЛЛ положен принцип индивидуального подбора терапии в зависимости от характера и тяжести клинической манифестации В-ХЛЛ, соматического статуса больного, а также возможностей проведения курсовой терапии в полном объеме. Современная терапевтическая тактика должна основываться на доказательных данных. Большинство данных, которыми мы располагаем, получено у первичных больных, а также у пациентов с 1–2-м рецидивом. В связи с этим в рекомендациях обсуждается терапия первой линии и 1–2-го рецидива.
